I am writing to bring your kind consideration towards my distress. I am working as (job designation: Assistant Accountant) in your esteemed place since last two/three/more years. This is to inform you that my mother/father is in bad state of health, he/she is suffering from severe heart problem (show your causes/disease name…) and doctor has prescribed him/her cardiac tests.
