It is informed that I have been assigned to take an examination of O-level (Physics) (Subject name and department name) from British Council (Institute name), on (date) at (Address or branch name). I have taken all previous exams without any problem. As a matter of fact, I have been suffering from a cardiac disease (Problem name) since the early days of life. As I felt severe pain in the heart at the start of this month, Therefore, I decided to visit a doctor.
