Sir,
It is courteously submitted, that I participated in XXth annual summer internship program, (Year…) held in your esteem institute from (date…) to (date…) and was issued certificate of participation.
Unfortunately, while preparing Photo copies of the certificate by the operator of Photostat machine accidentally a few drops of water got spilled over the certificate. Due to this the writing in ink on certificate got partially disfigured.
It is requested that I may kindly be issued a new duplicate certificate urgently, because of its requirement for my new semester.
