It is stated that I have completed my Bachelor’s Degree (Degree name….) this year from this prominent (institution/university name…..). I have got the transcript and all the clearance dues as well. Sir, this is to inform you that unfortunately I have lost my ‘Provincial Certificate’ showing my marks, grade and percentage. This certificate is of utmost importance for taking admission in any other institution for higher education.
Sir, I respectfully request you to kindly issue my duplicate ‘Provincial Certificate’ as in order to apply for my Masters (degree name…) I only have time of “yy” days. Within this duration, I have to submit my documentation. I have attached the copy of University ID Card with the application and all other necessary documents.
